Most pages selling this stuff wave at "cellular health" and move on. Here's the actual mechanism, in plain English, and then what the human trials found — including where they came up short.
Everything hinges on one molecule: NAD+ (nicotinamide adenine dinucleotide). Every cell you own uses it, and levels of it fall as you get older. NMN — nicotinamide mononucleotide — matters because it is the direct precursor: one enzymatic step (via the NMNAT enzymes) converts NMN into NAD+. That's the whole premise of the supplement. Take NMN, give the body the raw material for NAD+.
NAD+ is the shuttle your cells move energy on. When you break down glucose (glycolysis), run the Krebs cycle, or burn fat, each of those reactions strips electrons out of the fuel — and NAD+ is what catches them, becoming NADH.
NADH then carries those electrons to the electron transport chain inside your mitochondria, drops them off, and comes back as NAD+ ready to go again. The chain uses them to make ATP: the actual currency your muscles and brain spend.
So NAD+ isn't a fuel — it's the conveyor belt. If the pool of it shrinks, fuel is still there but less of it reaches the point of being spendable. That's the reason the "tired despite eating fine" complaint keeps coming up in NAD+ research.
NAD+ has a second job that has nothing to do with energy: a family of enzymes literally consume it to do maintenance work.
The sirtuins (SIRT1–SIRT7) are the famous ones — they cannot function without spending NAD+, and they regulate mitochondrial biogenesis (through PGC-1α), stress responses and how DNA is packaged. PARP enzymes spend NAD+ repairing damaged DNA. And CD38, which also burns through NAD+, becomes more active as you age.
That's the squeeze at the centre of the whole field: as you get older, demand for NAD+ goes up while supply goes down. Less left over for the sirtuins and the repair crew. Topping up the precursor is an attempt to relieve that squeeze — which is the theory, not a proven outcome.
Muscle insulin sensitivity improved — but the NAD+ increase itself wasn't detected. A 10-week randomised, double-blind, placebo-controlled trial in postmenopausal women with prediabetes found insulin-stimulated glucose disposal and muscle insulin signalling both rose on NMN and didn't on placebo. The authors were straight about the caveat: they could not detect a change in muscle NAD+ content.

Aerobic capacity improved in runners, dose-dependently. 48 amateur runners, six weeks, randomised into 300mg, 600mg, 1200mg a day or placebo alongside training. The higher doses improved how well muscle took up and used oxygen. No adverse effects were reported up to 1200mg a day.

NAD+ metabolism rose and it was well tolerated — the headline outcome missed. 36 healthy middle-aged adults took 250mg a day for 12 weeks. NAD+ metabolism went up and there were no adverse events, but arterial stiffness only trended downwards; the difference against placebo was not statistically significant.
